The aims of Social Prescribing Connect people to services and support in local places Reducing frequent and perhaps unnecessary attendance at GP surgeries Addressing the wider determinants of public health Improving community cohesion

The GP’s 6 GP surgeries on board In Worthing Lime Tree Surgeries Victoria Road Surgeries Worthing Medical Group and Adur Harbour View, Shoreham-by-Sea Manor Practice, Southwick Northbourne, Shoreham-by-Sea

Going Local so far Creating links to local Voluntary and Community organisations Lots of organisations expressed an interest in signing up to be part of the Service Directory Agreement forms and information sharing

The Reasons for Referral Social Isolation Money, Debt and Benefits Housing Employment & Training Digital Inclusion Lifestyle factors

Our first meeting We allow someone the time to talk through their concerns and worries Having up to an hour with someone can give a person this time to unload We will then talk through our service directory and create a plan together.

Where you fit in? We are not just a signposting service – we will assist people to access your service We want to know your services well Accessing services to benefit from them OR being able to help out and volunteer to benefit a service.

Initial responses and reporting Initial needs have been around; benefits, emotional support and social isolation We’ve had 30 referrals since the start of the month. Most of which have been booked in Case studies…

Evaluation and the hope for this to be continued into the future Evidence is being collected already Our MATS software can be simply adopted into a new GP surgery. It can easily be rolled out to a wider area
