Lesser Trochanter Osteoplasty for Ischiofemoral Impingement Jill A. Goodwin, M.D., Anikar Chhabra, M.D., Karan A. Patel, M.D., David E. Fig 1 T2-weighted fat-suppressed axial MRI of the pelvis with right ischiofemoral impingement. The ischiofemoral space (red line) is the smallest space between the lesser trochanter (arrow) and the ischium (arrowhead). The quadratus femoris space (blue line) is the smallest space between the iliopsoas tendon and the hamstring tendons. Edema of the quadratus femoris muscle (star) is also evident. (MRI, magnetic resonance imaging.) Arthroscopy Techniques 2017 6, e1755-e1760DOI: (10.1016/j.eats.2017.06.047) Copyright © 2017 Arthroscopy Association of North America Terms and Conditions

Fig 2 Recommended operating room setup for a right hip arthroscopy, with the patient supine in traction boots, and fluoroscopy introduced from the contralateral side and perpendicular to the patient. Arthroscopy monitors (arrow) are at the head of the bed. The fluoroscopic monitors (arrowhead) are at the foot of the bed. Fig 3 Intraoperative outline of recommended portals for a right hip, with the first incision (arrow) 2-3 cm lateral to a line from the ASIS to the patella, and 3-4 cm distal to the greater trochanter (star). The second incision (arrowhead) should be approximately 6 cm distal to the first incision, in the same medial to lateral plane. (ASIS, anterior superior iliac spine.) Fig 4 Fluoroscopic image of a right hip showing the relationship of portals and instruments to the lesser trochanter (arrow). Separating the incisions by several inches will allow the surgeon's hands to avoid crowding one another. Fig 5 This is an image of the surgeon with instrumentation in the space around the lesser trochanter of the right hip conducting bursectomy. It is important that prior to externally rotating and flexing the hip, instruments should be inserted with the leg in internal rotation, in order to develop the proper soft tissue planes to ensure that instruments are deep into the femoral neurovascular structures. Fig 6 Endoscopic image of a right hip showing the iliopsoas tendon (arrow), which should be easily visualized after bursectomy. Arthroscopy Techniques 2017 6, e1755-e1760DOI: (10.1016/j.eats.2017.06.047) Copyright © 2017 Arthroscopy Association of North America Terms and Conditions

Fig 7 Endoscopic image (A) and fluoroscopic image (B) of a right hip with correct placement of the ablater or shaver (arrow) on the lesser trochanter with the leg in slight flexion and external rotation. This ensures that the surgeon is at the correct level for iliopsoas release and then subsequent lesser trochanter osteoplasty. The lesser trochanter (arrowhead) should be resected to the level of the surrounding cortical bone (arrow).
