Aims & Objectives Aim To provide an outline of how WCCIS is being implemented within a live integrated health and social care team Objectives To provide background and context to the proof of concept project To describe the benefits emerging from the proof of concept project To demonstrate the functionality being utilised in the Bridgend CRT To identify lessons learnt and the resulting next steps

Background WCCIS provides a “Once for Wales” solution Bridgend County Borough Council 1st implementing organisation Western Bay Optimal Model – integrated health and social care Bridgend Community Resource Team (CRT) based in Trem Y Mor: Structure Common Access Point (CAP) & Multi Disciplinary Triage (MDT) Therapies – Reablement, Occupational Therapy, Physiotherapy, Speech & Language, Dietetics Acute Clinical Team (nursing element) Social Work Assistive Technology Dementia Support Pharmacy Purpose Supporting people to remain at home Minimising unscheduled admissions to hospital Bridging the gap between inpatient/community care and supporting seamless discharges Focus on enabling and reabling individuals to remain as independent as possible
