Flinders CCM Upskilling Workshop Chronic Disease Wellness Programs Gold Coast Health Queensland Australia

What was the Issue Staff that had previously attended Flinders CCM 2-day workshop had not gained Competency Certificate

Aim Was to provide a 1/2 day Upskilling of Flinders tools for staff to review and gain the competency required level encouraged by Gold Coast Health Chronic Disease Wellness Programs (CDWP) Plan of session: Flinders Accredited Trainers met Rose Costa, Coordinator CDWP, Janine Paul, Dietitian & Judy Murrells Respiratory/Mungulli CNC What would work?

What would work? Role play of a previous Flinders assessment where PIH/C&R/P&G then client-centered care plan developed. Client was selected & reviewed by Janine (role play - client) & Judy (HP) client de-identified for purpose of training.

Planning Review of tools & principles of Self Management/SMART goals / Motivational interviewing & developing care plan Flinders tools for each participant

At the session Participants: Heart Failure CNC/Heart Failure CN's x 3 Pharmacist/Psychologist Exercise Physiologist. Review of tools Role play attended - participants completed PIH/C&R/P&G development care plan Interesting differences high lighted within the MDT Participant expectation to complete all tools each scoring & recording as the health professional Completion of care plan to be submitted on the day or within 2 working days.

At the session Janine played the (client) role so well as the HP taken back to original client assessment & felt same empathy & emotion with her story Participants completed flinders tools from the role play

Outcome 100% participants submitted care plans for critique The session was valued by the staff. Trainers felt a good way to support busy clinicians to increase completion & gain competency certificate. Addition to the outcomes: an evaluation report was completed by Janine Paul Its conclusion and findings were:

Conclusion The Flinders Chronic Condition Management’ Workshop Refresher was a successful training event which all participants were able to gain competency and/or update their skills with utilising the Flinders Program tools.

Recommendation To conduct two ‘Flinders Chronic Condition Management’ Refresher Workshops twice a year to: 1. allow staff to update their skills with using the Flinders tool and/or 2. to all achieve competency in the Flinders Chronic Condition Management’ tools/processes. By doing this it will benefit our clients and demonstrates the Chronic Disease & Post Acute Programs, Gold Coast Health is committed to delivering an evidenced based self-management program which produces positive client outcomes.
