Group Accident –Colonial Life The prior “Personal Accident Plan” is being replaced by this plan. Off the Job Accident coverage Sickness Hospital Confinement benefit included Guaranteed issue (no medical questions) Benefits paid based on a schedule of benefits coinciding with the injury and the treatment received (following submission of EOB’s) Wellness Benefit (for 1 covered screening per calendar year per covered individual) $50 for employee, spouse, and child(ren) Coverage options include: Employee only Employee + Spouse Employee + Child(ren) Family
