About Christian Care Ministry Authentic Christian Community Christian Care Ministry is committed to serving the Lord and our membership with excellence. Headquarters, Melbourne, FL Founded in 1993, CCM moved to its current location on Eau Gallie Boulevard in Melbourne in October of 2014. What is Medi-Share? Affordable alternative to health insurance (Approx. half the cost!) Community of believers voluntarily share one another’s medical bills Members can choose any provider in or out of network PHCS PPO Network with over 700,000 providers Unlimited annual and lifetime maximums with few sharing limitations* No open enrollment - available year-round Available in all 50 states * $50,000 per person during the first month, $125,000 maternity limit

Sample Member Card Includes: Membership numbers Initial charges per Office Visit and Hospital, Emergency Room Visit ($35 and $200 respectively) Contact numbers for members and provider web site PHCS logo signifying the present PPO contract *$200 ER fee

What Happens at a Provider’s Office? Medical event occurs Member selects a Network Provider* Private Healthcare Systems (PHCS) offers a PPO network with more than 700,000 providers nationwide Member shows Medi-Share ID card and pays Provider Fee $35 initial charge for doctor visits $200 for an emergency room visit Provider notifies and sends bills to Medi-Share *Members may select any provider of their choosing, including out-of-network providers, but in doing so may not benefit from discounting of medical bills as with an in-network provider. *$200 ER fee

What Happens to Medical Bills? PHCS negotiates discounts. An average adjustment of 30–35% by staying in-network. Provider Fee of $35 or $200 is also deducted from the total bill. Medi-Share processes the bills for sharing. Medi-Share notifies member of any amount they must pay directly to meet their AHP. For any amounts to be shared, Medi-Share transfers sufficient funds from other members’ accounts into the billed member’s account. Medi-Share notifies sharing members of the recipient of their monthly share amount. Share amounts are used to pay providers. Once the dollars are transferred, Medi-Share processes payment to the doctor or hospital from the member’s share account. *$200 ER fee
