SOP 3: Submit New User Request This SOP is the final step to gain full access to the hospital’s network **Please read each slide carefully and in it’s entirety!!**

YOU DO NOT NEED TO EMAIL ANY FORMS TO YOUR DEPT. HEAD/SUPERVISOR Filling Out The SharePoint Request Form Pt. 1 Now that you have the completed the required forms and training, it’s time to complete the backbone of your Network/CIS Request. This portion of the request will be used to forward your request to the people that need to sign it, and you will enable you to check the progress of your request at any time. THIS FORM WILL AUTOMATICALLY FORWARD YOUR FORMS TO YOUR DEPT HEAD AND SUPERVISOR YOU DO NOT NEED TO EMAIL ANY FORMS TO YOUR DEPT. HEAD/SUPERVISOR Right click and open in new tab CONTINUED ON NEXT SLIDE IN PT. 2

Filling Out The SharePoint Request Form Pt. 2 You will be brought to this page showing the Request Form. - Before you start filling this page out, hit “F10” on the keyboard. A small menu will pop up at the top left of the page. From the menu , select “Tools”. Then select “Enterprise Mode”. This we reload the page, and you are now able to start filling out the form. A guide on that will continue on the next slide.

Filling Out The Sharepoint Request Form Pt. 3 Fields within the red boxes are required fields. Fill them out accordingly - Important Notes: - When entering dates on this form, ignore the guidance below the date box, and type in the date as MM/DD/YYYY, select the small calendar icon, then select the date from the calendar. Do this for each item that has a calendar next to it. - For the ‘Type of Request’ field, use the drop down menu to select the correct type of request. If you are not working with clinical systems, you will only need ‘Basic’. If you will be working with clinical systems (AHLTA/CHCS/Essentris), then choose the ‘Basic & CIS’ option from the drop down. If you are trying to extend you CIS access, select “CIS”. - If you require share drive access, then mark Sharedrive access “Yes”. If marked “Yes”, then annotate in the notes your Directorate and Department. Continued in Pt. 4 on Next Slide

Fill them out accordingly, AND BE SURE TO READ THE GUIDANCE BELOW EACH REQUIRED FIELD Important Notes: - When entering dates on this form, ignore the guidance below the date box, and type in the date as MM/DD/YYYY, select the small calendar icon, then select the date from the calendar. - For “CIS Access”, if you will be working with clinical systems, check all boxes that apply to you and the clinical systems you require. - “User’s email” is your official @mail.mil email address - IT IS EXTREMELY IMPORTANT THAT SUPERVISOR EMAIL MATCHES WHAT YOU PUT ON YOUR SAAR-N FORM, AND THE DEPARTMENT HEAD EMAIL MATCHES WHAT YOU PUT ON YOUR CIS REQUEST FORM. Filling Out The SharePoint Request Form Pt. 5 YOU DO NOT HAVE TO EMAIL ANY FORMS TO YOUR DEPT. HEAD OR SUPERVISOR!!! 1. Now that the form is filled out, its time to upload all required documents to this request form. - At the top left hand part of the request, select “Attach File” 2. - You will have to upload each document separately, one at a time - Select “Browse”, then navigate to your (P:) Drive and select one of the required forms. - Repeat this process until all required forms and documents are uploaded. 3. Here is a list of all required forms for Network access and CIS access Completed SAAR-N Form HIPAA & Cyber Awareness Training certificates. Signed AUP (USNHOKI 5238/1) CIS Form (CIS ONLY) JLV VIEWER (CIS ONLY) 5. Once all required documents are uploaded, click “Save” on the bottom right hand side of the Network Request Form. YOU DO NOT HAVE TO EMAIL ANY FORMS TO YOUR DEPT. HEAD OR SUPERVISOR!!!

**THE NEXT FEW SLIDES ARE VERY IMPORTANT** Viewing Your Request Pt. 1 **THE NEXT FEW SLIDES ARE VERY IMPORTANT** Now that you have completed the network request package, it’s very important for you to view it at least daily to make sure it’s moving along. It’s also important to check on it to see if there are notes on it saying something is incomplete or missing. To view the progress of your request, follow these steps: Navigate to the hospital’s Sharepoint page. Click on the “Help” drop down menu, and select “ITD”. Select “Check Status of a Request” CONTINUED ON THE NEXT SLIDE

Viewing Your Request Pt. 2 This page will load and your Network Request will be displayed HERE To check the status of your request, click on your LAST NAME CONTINUED ON THE NEXT SLIDE

Viewing Your Request Pt. 3 This page will load, and you will be able to see all the details regarding your request. Pay special attention to the “NOTES” section. This section will let you know if there is anything missing or incorrect. The Signature Fields will let you know who still has to sign your attached documents.
