WellCareTracker™ – Linking Child Care with Medical Homes Jerold M. Aronson MD MPH FAAP PA – American Academy of Pediatrics

Pennsylvania Chapter - American Academy of Pediatrics

ECELS-Healthy Child Care PA was conceived in 1988 as a program of the PA Chapter of the American Academy of Pediatrics to link the resources of health professionals, early education professionals and state wide agencies to improve quality in PA early education and child care facilities.

Agenda Health Record Issues Health Record data feedback improves preventive care performance Demonstrate WellCareTracker™ Brainstorm during panel discussion

Healthy People 2010 Achieve and maintain immunization 90% 4:3:1:3:3:1 (19-35 mo.) 95% 4:3:1:3:3:1 (19-35 mo.) Child care Blood Lead – children age 1-6 yrs. 0 %

Educator Health Challenges

PA Regulations DPW – all programs must document UTD preventive care services (immunizations & screenings) within 60 days of enrollment (55 Pa. Code § 3270.131) DOH Reporting of Communicable Disease (Chapter 27.7) – enrolled children will be UTD for immunization and child care program will report UTD status annually.

Medical Home Challenge Preventive Care - Today

WellCareTracker™ 6 years of statewide data trends > 95,000 health records Web-based application = www.WellCareTracker.org Accessible with any PC connected to the Internet. Password protected/private Added item on access of child care providers to computer & Internet to 2002 child care provider survey. Surprised to see how widespread computer use is among child care providers. Two thirds of centers have computers and nearly half use the internet. FCCH even greater computer use: 75%. FCCH internet use is 41%. Widespread use of computers makes use of computerized tracking of preventive health services realistic for many early education programs.

Health record data feedback increases the number of preventive care services received by children attending licensed childcare in PA.

The variability of age of the child complicates the educator’s challenge to keep children UTD during the enrollment year.

2,798 PA Licensed Child Care Centers: 13,675 child records checked between 7/02 and 4/03
