Confidentiality Agreement

AMITA Health Confidentiality Agreement I understand that in my capacity as an employee, volunteer, or independent contractor of AMITA Health or its affiliates (collectively, AMITA,) I may have access to confidential and proprietary information (Confidential Information) concerning patients, staff, and operations of AMITA. Confidential information shall include, but is not limited to, any information (written, verbal, or other form) pertaining to patients, employees, business practices, strategic plans, financial information, or other information regarding the operations of AMITA. I agree to abide by the following provisions:   I will only access Confidential Information on a “need-to-know” basis using the minimum-level of information necessary to carry out my duties or assignments. I will not disclose Confidential Information to any third party except as required to carry out my duties or assignments in compliance with AMITA policies and procedures and applicable law. I acknowledge and agree that if I receive a computer system unique user ID and password, I will not share my user ID or password with any other individual or third party or otherwise allow access to any AMITA computer system. I understand that computer access may be monitored for compliance.

AMITA Health Confidentiality Agreement Confidentiality Agreement Continued: I will not remove patient information or other Confidential Information regardless of medium or format from the health care facility without the approval of AMITA. I will promptly notify my manager or facility Privacy Officer of any activity that may involve an inappropriate disclosure of Confidential Information. Consequences of a breach of Confidential Information, including patient information, be it intentional or not, may result in corrective action up to and including termination and/or possible liability in any legal action arising from such breach.
