Introduction We would like to extend our sincere appreciation for your involvement in piloting our new Workplace Safety Audit Mobile Application (powered by Origami). We firmly believe in advancing technologies centered around improving the user auditing experience by simplifying the time spent by your Associates. The primary objective of this pilot is to test the usability of the application; it’s navigation, clarity of questions, tooltips, attachment capabilities, etc. It is essential that you take an un-biased look at the current status of your marketplace for the self-audit. 100% scores are not beneficial to the unit or to the safety of the associates. There are NEW questions and a NEW scoring scale with this version of the audit, so please ensure you read the ENTIRE question prior to answering.
Downloading the App In order to access the app on your mobile device or tablet you will need to download it from the Apple or Google Play Store.

App User Instructions Upon downloading and launching the app, you will then need to Enter the Account Name: Compass Then you will enter the 6-Digit Code for Eurest: 266001 Once Signed in, you will then be taken to your forms. Before beginning, you will need to refresh your forms by clicking on the circle with the arrow in the upper right corner, then you will select to access your available forms by clicking on the + sign. < Dining Safety Audit Done App User Instructions Once you initiate the audit you will need to fill in the blanks. To input the location that you are auditing, you will click and put in your unit number. Your unit should show up, click on the proper unit. You will perform audit and select the proper response for each question on the audit (meets expectations is no longer an option). You can see Tool Tips to see exactly how to provide response to each question by clicking the .
App User Instructions On the bottom right of the screen you can select the camera icon and take a photo or video or attach one from your camera roll. Corrective actions will automatically populate for all questions that were marked as “needs improvement” when the audit is submitted. Upon answering ALL questions, you will then select “DONE” in the top right corner. Then the box will pop up for you to submit. You will select the top option to SUBMIT THIS FORM.
Action Items Upon submitting the audit, you will receive an email with the PDF audit attached along with a final score. Action items will be auto-populated for any questions that were marked as “Needs Improvement”. An email for each question with an action items will be sent to the Responsible Email listed at the beginning of the audit. Within 30 days, you will need to update the status of that action item using the links in each corresponding email. After 30 days, the link will expire.
Recap Units without a cell phone or tablet and cannot download the app should complete the QSI using the excel spreadsheet. For any questions, reach out to your Regional Safety Champion. The deadline to submit your safety self-audits is before May 26, 2017.
