Use warm-ups/bell work to allow for smooth transitions and maximize learning time. Consider using please and thank you on your written instructions (on the board and on assignments).

Repetition is the key. For a child to learn something new, you need to repeat it an average of 8 times. For a child to unlearn an old behaviour and replace it with a new behaviour, you need to repeat the new behaviour an average of 28 times. (20 times to undo the old behaviour and 8 times to learn the new one.)
