Why is LDL control a challenging measure? “Every three month” measure Dependent on patient commitment to care Medication adherence Price Side effects Drug interactions Lifestyle change Follow through with lab work

Addressing Uncontrolled LDL Quality Team (NCM, Quality Asst, Quality Clerk) Identifies outliers by running report for review by PCP Contacts uncontrolled patients on case-by-case basis MD-targeted interventions Monthly CSI MD meeting to review performance and data Stress target Encourage referrals to NCM, nutritionist
