Whole Systems Integrated Care (WSIC) Over 65s London Care and Support Forum 11th May 2016 “I want my care to be coordinated and not to feel afraid” COMPREHENSIVE & COMPASSIONATE CARE

The WLCCG model

Whole Systems model of care GP Federation and local GP practices 46 in total Whole Systems Patients aged 65 and over GP - Out of Hours and 111 Adult Social Care co-located with team in MDT’s and HUB’s Community, Acute and Mental Health providers Integrated Care Centres funded by WLCCG Voluntary organisations (KCSC, Age UK, Open Age and more)

The response to our offer in action ‘Additional time in consultation allowing a full discussion on problem.’ ‘Ease of access. GP has more time to cover all aspects of my care. St Charles is hugely more patient friendly that other hospitals.’ ‘Time to express and listen to my needs.’ ‘The time spent with the doctor. I was able to explain my situation without being rushed. I felt the doctor had time to listen and explain to me what I needed to know.’ ‘Carry on with this new system. It is also very pleasing to know subconsciously that should something happen, all the expertise is at hand.’ Patient comments from the survey November to Mid Jan 2016 ‘The doctor gave me a handout of a summary of the goals we wish to achieve and how we can achieve them. There's hope!’ ‘I went home feeling a lot better than when I came in.’
