Simpler. Stronger. Social Care In Swindon we are changing the conversation. Building a simpler, stronger approach to adult social care, that improves the lives of the people of Swindon. Our aim is to find out what’s most important to the individual, to support creative solutions that promote well-being and independence.

Our Journey to Support Adults in Swindon… 1.  Helping you to help yourself   Accessible, friendly, quick, information, advice and advocacy, universal services for the whole community, prevention 2.  Helping you when you need it Immediate help, minimal delays, no presumption about long-term support, goal focussed 3.  Helping you to live your Life – ongoing support for those who need it Self- directed, personal budgets, choice and control, highly personalised

We now effectively signpost over 70% of calls at first contact. Adult Community Team The team are helping people to be the best they can be, as fast as possible. Linking individuals, families and carers to community based solutions. We now effectively signpost over 70% of calls at first contact.

The Acute Discharge Team, based at Great Western Hospital are promoting independence to get people home.

75% of people that have been supported over the past 6 months are back to full independence. The Reablement Team The team are developing effective reablement support by personalised independence plans, daily reviews and a multi-agency approach to be the best they can be and remain in their own home. The reablement service now has capacity for double the number of service users

The Assessment and Review Team The team is changing the assessment conversation. We want to understand what is most important to the individual. What is working, what is not working? What resources does the adult need to achieve his/her outcomes? Average time taken for a new service users to be allocated to a social worker reduced from 4 months to 15 days.
