Bury Neighbourhood Engagement

Welcome and Introductions Attendees to introduce themselves

Neighbourhood Engagement Meetings Previous CCG ran sector meetings have finished Neighbourhood meetings have been set up in all five neighbourhoods: North East West Whitefield  Prestwich Purpose- to provide a forum for health and social care professionals working in a neighbourhood to have dedicated time to discuss key issues Time will be given at the Feb meeting to discuss how neighbourhood meetings will work post April. This will also be briefly covered at the end of this presentation. Payment to practices will continue via the CCG for Jan and Feb- new arrangements (TBC) will be in place from April.

Neighbourhood Team Progress Shared care plan Single template for a care plan has been agreed Final IT solution still to be determined Additional neighbourhood roles Funding agreed for the following per neighbourhood: Neighbourhood lead- 1wte Administrative support- 1wte Neighbourhood connector- 1wte Pharmacist- 0.6wte Therapist- 0.6wte Health/Social Care professional- 1wte Above in addition to GP chair – 0.2wte Work arounds in place for sharing care plan- e.g. Microsoft Teams- single read/write care record wont be in place for the 1 April Agreement gained at LCA Board to flex roles based on local need. Lead, admin and connector are non-negotiable. Recruitment to take place over Q4 18/19. 

Neighbourhood Team Progress MDT Bury model designed 'Red lines' Screening of referral by more than one person Use of consistent paperwork templates (to be developed on a Bury footprint) Weekly MDT meetings to take place Core team must consist of: Neighbourhood lead, Neighbourhood Connector, GP, District Nurse Lead, Social Care Lead Additional members to be appointed as required Risk stratification to be used First meetings to take place in all areas during February  Bury model to be reviewed 90 days after roll out Single model across Bury Local flex within the 'red lines' Stress that what is in place from the 1 April wont be the final way of working- 19/20 is around building relationships, trying new ways, honing the best way of working All neighbourhoods will be given opportunity to comment
