Musculoskeletal Condition Management Program Available now! Health Advocate’s Musculoskeletal Condition Management Program leverages a broad range of expertise in collaboration with workplace resources and vendor relationships. Through a unique combination of outreach, engagement and personalized support, we keep employees safe, healthy and on the job: Personalized support—the keystone of all of Health Advocate programs Nurse Coaching, Health/Lifestyle Coaching, Health Advocacy, Second Opinion Support, Mental Health Support Engagement and direct outreach—Driving participation Telephonic outreach, HIPAA-compliant emails, Member Engagement Platform Advice, mobile push notifications, worksite promotional materials Optional incentive management Integration with disability vendors and health plans

Hypertension Kickstart Coaching Program Available now as part of the Wellness Program The Health Advocate Wellness Program provides employees access to Wellness Coaches to help them improve their health in a variety of areas, including better control of blood pressure. We’ve created a Kickstart Coaching Program consisting of 3 telephonic coaching sessions with a curriculum specific to blood pressure. During the coaching sessions, employees and their Wellness Coach will: Discuss lifestyle and its effect on blood pressure Identify areas to improve and make changes Collaborate to set goals and create an action plan Find workable solutions to challenges that may interfere with meeting goals Establish a plan to help self-monitor and track blood pressure

Diabetes Prevention Program Coming in 2019 Through one-on-one support to manage glucose, Health Advocate’s Diabetes Prevention Program utilizes evidence-based approaches to reducing at-risk members’ likelihood of developing Type 2 Diabetes. Health Advocate experts will help members: Learn and adopt healthy lifestyle change to better manage glucose Develop a plan to help maintain a healthy weight Find ways to fit exercise into their schedule Create a balanced nutrition plan Monitor and understand blood sugar levels Understand the tests and treatments needed to manage glucose and know what questions to ask their doctors
