Why Patient stories Patient team members and stories To find out about patient experiences of their local health service. To inspire staff and teach future doctors and nurses. To learn what can be improved and how it’s been done. To help spread the Campaign’s message and achievements. Patient team members and stories

How do we collect the stories? To use a questionnaire to help patients to tell us their story in their own words. When a story collected via a questionnaire is particularly relevant to the Campaign’s work we hope to follow up with a filmed interview. Patient team members and stories

The questionnaire: Describe what happened. What was the main good thing about your experience? What was the main bad thing about your experience? What would have been the one thing that would have made a difference? What would be the one message you would tell your Local Health Service after your experience? Patient team members and stories

The filmed interviews Patient team members and stories Only stories with direct involvement with areas covered by the 1000 Lives Campaign will be considered for filming. We will contact the story-teller to ask permission to film them telling their stories. The patient will be filmed telling their stories in their own words. They will be filmed in a place convenient to the patient by a professional film crew. No one involved in the patient’s experience will be involved in the interview. They will be used during local, national and international events. A new consent form will be required. Patient team members and stories
