Stakeholders Prioritize Six Key Elements When Developing Strategies to Address Population Health Issues Although approaches to population health vary somewhat across stakeholders, they possess six common elements. Each stakeholder focuses on these elements, from basic to advanced levels. Although lack of resources will limit stakeholder progress, they typically agree that success depends on strong outpatient care teams and ability to analyze and respond to data. Individual stakeholders typically excel at some elements, while performing poorly on others.
