What is CARE? Cooperative Agencies Resources for Education (CARE) is a state funded educational program established to assist single, head-of-household students receiving cash aid for themselves or their children with resources to support the student while achieving their academic goal.

Program Eligibility Student must have an open case number with CalWORKs, TANF and be receiving cash aid for themselves and/or their children Have at least one child under the age of 18 Be single and head-of-household Be at least 18 years of age Must have applied for Federal financial aid Student must be in the EOPS Program and be in good standing to be eligible to apply for CARE.

Eligibility for CARE Services To receive CARE services you must: Attend the EOPS orientation (as required) Attend the CARE Orientation (as required) Follow CARE policies as outlined in the CARE handbook Must have financial unmet need Attend an Intake appointment with the CARE Coordinator and provide the following: completed agency certification completed child care verification form

CARE Services (Based upon available funding) Child Care Grants - 3 disbursements (must attend workshops) between $800- $1200 this semester Gas Cards – 3 gas cards this semester between $25-60 each (must attend workshops) Auto Repair/Essential Maintenance Reimbursement (up to $200) per semester. CARE Textbook grant Meal Card ($25.00) Required course supplies Referrals

Child Care Grants You must attend a workshop in order to receive your child care disbursement. You can find different workshops on the MSJC website at https://calendar.msjc.edu

Child Care Grants Continued Make sure that the form is completely filled out and signed by the presenter. Verifications without the “Apply to:” circled will be discarded and you will not receive your child care grant.

Gas Cards You will receive three (3) gas cards for the Spring 2019 term. You must attend a workshop in order to be eligible for the gas card. Disbursement dates are located on the Spring 2019 Transmittal dates document. The last day to pick up all gas cards for Spring 2019 is May 18, 2019.

Automotive Repair/Essential Maintenance Reimbursement Request You must submit your pre-approval request with: Copy of your valid driver’s license Copy of your current car registration Must be pre-approved. Once approved, have required repairs/maintenance completed. Submit Original proof of payment itemized receipt, clearly indicating how the care repairs were paid for the receipt must indicate form of payment. If cash payment was made, the receipt must show the amount that was paid and if there was change, etc. The balance should show zero.

Automotive Repair\Essential Maintenance Reimbursement Allowable reimbursement expenses Oil Changes Tires Vehicle Emergencies Smog Fees $200 maximum reimbursement per semester Last day to submit reimbursement request form is April 20, 2019. Last day to submit receipts for reimbursements is April 30, 2019.

CARE Book Grant If GAIN pays for your books, you will not receive a book grant from EOPS or CARE. Once your EOPS book grant is spent (or you cannot purchase another book with the amount you have left on the grant), ONLY THEN can you begin the CARE book grant process: You will need to fill out a “Textbook Request/Reimbursement Form” that is in the eops office and online You will need to provide a copy of your current class schedule A “book quote” – you can retrieve a book quote from the MSJC Bookstore website-please do not go to the Bookstore to get a quote THIS PROCESS CAN TAKE UP TO SEVEN (7) BUSINESS DAYS SO PLEASE PLAN ACCORDINGLY For those eligible, CARE will only purchase required textbooks from the MSJC bookstore (new or used)

Meal Cards One $25.00 meal card will be provided to all CARE students that meet the eligibility. No workshop needs to be completed for the card Meal Cards will be provided at the same time the 2nd gas card will be distributed. Cards may be only be used at the MSJC cafe You must designate which campus you would want your meal card for when you do your intake with the coordinator. Meal Cards are campus specific.

Required Course Supplies Request supplies through GAIN, if not applicable, request through EOPS, last resort is CARE.

Documentation To verify your eligibility in the program you will need to complete and have verified the following two documents Agency Certification Form Child Care Verification Form

Agency Certification

Agency Certification Must be completely filled out DPSS representative at front desk can fill it out for you. On campus CALWORKS program has a DPSS representative Andrea Funston, her availability is Monday and Thursdays from 10 am – 4 pm in the San Jacinto Calworks office. Provide document to CARE coordinator during Intake appointment If document is not completely filled out DO Not come in for an intake.

Child Care Verification This form is to verify your child care provider information You must fill out the top portion of the form. The bottom portion of the form is for your child care provider to fill out. Your child care provider does not have to be a child care center. Please do not leave any question unanswered. Provide completed form to CARE Coordinator during intake appointment.

CARE Intake Attend a CARE Intake with the CARE Coordinator during the scheduled time frame Intake range from 10-20 minutes Intakes are done on a walk-in basis ONLY! Students will be evaluated for eligibility at the time of the intake and will be notified if they are eligible for the program at that time.

Next Steps Have the Agency certification and child care verification form completed/filled out prior to your intake. Complete your intake with CARE coordinator on the designated dates and times this must be done by February 23, 2018. Once approved, start working on your workshops.

Deadlines CARE Intake-Last day is February 23, 2018 SJC – January 10th 9 am – 10:30 am; January 16th 2 pm – 3:30 pm MVC – January 16th 10 am - 12:30 pm; January 17th 3 pm – 5pm TEC – January 24th 2:30pm – 4 pm Please bring the following: Agency Certification Child Care Verification Form CARE Book Account-Last day to use the account is April 27, 2018. All book accounts will be closed Automotive Repair/Essential Maintenance Reimbursement Request-Last day is April 20, 2018 Last day to request auto repair (pick up form from CARE Coordinator) Automotive Repair/Essential Maintenance Receipt -Last day is April 30, 2018 at Noon. Last day to submit all paperwork to process your auto repair reimbursement

Reminders If ANY services are abused, you will be removed from both the EOPS and CARE Programs You must comply with the EOPS Mutual Responsibility Contract to receive CARE services Please remember to comply with your required three contacts with your EOPS counselor: At San Jacinto, Menifee, TEC or San Gorgio Pass campus When you meet with the CARE Coordinator this DOES NOT COUNT as an EOPS contact.
