Lorn & Oban Healthy Options Ltd Self Management: A Partnership Approach

Collaboration from the start, collaboration at the heart. BACKGROUND TO LOHO Charitable organisation established in 2011 by community activators, Atlantis Leisure, health & exercise professionals. Committed to improving health & wellbeing through social prescribing, education, advice and interventions from experienced exercise professionals. Collaboration from the start, collaboration at the heart.

If the problem is in the community, the solution is in the community. There are 150 different organisations in and around Oban either directly, or indirectly improving our health. Versus Arthritis Oban Lorn & Islands Health & Wellbeing Network West Highland Housing Association Multiple Sclerosis Therapy Centre Lorn and Oban Healthy Options North Argyll Carers Centre Health and Social Care Partnership Atlantis Leisure Centre Roses Charitable Trust Community councils 150 organisations: Not just sports clubs and gyms, but art, craft, music, drama and social groups They all improve our health, reduce isolation and loneliness, and get us active in some way. If the problem is in the community, the solution is in the community.

40% of our community live with one or more chronic medical condition Health Improvement activities and education through social prescription. Delivered by highly qualified professional staff. Reablement Programme: Partnership working with Physiotherapy as part of the Oban Living Well Project 1:1 sessions to deliver a tailored programme at home or in the community. Mainstream programme: 1:1 Consultation Tailored exercise & activity programme Move well classes Health & Wellbeing education sessions Re-wild walking group. Community Outreach Programme: Appin, Easdale, Dalmally ,Taynuilt Strength & balance classes Tai Chi Community Move Well Classes Self Management Courses MS Class Argyll Networks Programme: Working with socially isolated and/or vulnerable groups Indoor/outdoor activities Move well Classes Link working Demographic projections from 2010 – 2035 suggest75% more people aged 75+

FUTURE DEVELOPMENTS Social Prescribing Project with Scottish Communities for Health and Wellbeing ALLIANCE funded Self Management 11 programmes across Argyll & Bute Falls Prevention classes in partnership with LIDGH physiotherapy service Partnership working with HSCP to assist in the development of the Community Link Worker Role One of 10 third sector organisations in Scotland (and 5 in NI with 20 sub groups) to be part of the social prescribing project One of 26 organisations in Scotland to receive funding from Health & Social Care Alliance Scotland (ALLIANCE) to deliver 11 Self Management Programmes, and 2 Chronic Pain Events across Argyll and Bute Funding till March 2019 to build evidence base for social prescribing and link working in Argyll and Bute. Project lead is Jackie Barron Current high risk and low risk falls classes run in physiotherapy dept to be delivered in the community. Assessment from physio service, delivery of 12 week programme by LOHO staff. Key Partner in developing and implementing Oban Health Town.
