Strategies for improvement: Access Community Health Network PSS Quality Improvement Presentation: Support Group Rationale: To increase the number of clients at support group at Madison Family Health Center Baseline data 2016-2017 grant year: One client attended support group at Madison Strategies for improvement: Change from bi-weekly meeting to quarterly meeting Cancelled support group at Grand Boulevard Health and Specialty Health Center to allow clients to attend at Madison Flyers for promotion Survey conducted to gather patient interest in support group and assist in choosing topics Different topics for each support group session

Results of new support group format Access Community Health Network PSS Quality Improvement Presentation: Support Group Results of new support group format August 23rd support group 6 clients attended October 25th support group 7 clients attended Increased client interest in spiritual care
