Fig 1 Conceptual model of the pathobiology of PGA. A variety of triggers induce alterations in structural cell function (myofibroblasts and epithelial, smooth muscle, and mast cells) that engender AHR. Typically, patients with PGA manifest insensitivity to corticosteroid therapy. In a corticosteroid-insensitive manner structural cells also modulate airway inflammatory responses, such as chemokine and cytokine secretion and leukocyte trafficking.
