The Adirondack Health Institute is …. AHI is an independent, non-profit organization that partners with regional health care providers and community-based organizations to improve care, lower costs, and realize a healthier future.

What we do…. 1. Promote population health best practices and implementation strategies. 2. Manage programs to advance health; and 3. Ensure individuals have access to care.

PHIP Objectives Support and advance the Prevention Agenda, the State Health Innovation Plan (SHIP) and the State Innovation Model (SIM); Serve as a resource to Delivery System Reform Incentive Payment (DSRIP) program’s Performing Provider Systems (PPS)

NC PHIP Timeline Summer 2015 Fall 2015 Winter 2015/2016 Convene diverse stakeholders to create the 6 county North Country Population Health Collaborative (NCPHIC). Fall 2015 Create and maintain the HealthyADK regional website. Develop community health data infrastructure to support regional initiatives. Winter 2015/2016 Develop and implement regional Community Mobilization Teams using a collective impact model to improve health disparities.
