Building a Community of Caregivers Around Your Child For Life

PROVISION OF SUPPORT SERVICES FOCAL POINTS Home support is separate from housing and that additional services/supports that are not included in the Program Participation Points are subject to corresponding fees

Financial requirements for OLFI Regular Members (Service- Providers): A one-time membership fee in the amount of P 90,200.00 shall be required from Regular Members. This entitles the member to lifetime membership to OLFI.

To ensure active and regular participation in OneLife’s community programs, a Community Fee is charged for each member. The Community Fee is broken down as follows: 1. Program Participation which entitles the member to participate in monthly community meals. The amount is P3,000.00/annum. 2. MBH UpKeep Fee – this is used to pay MBH utilities and for maintenance. The amount is P4,800.00/annum. The Community fee amounts to P 650.00/monthly.

E. Seniority in Housing Queue – an OLFI Regular Member does not have to be interested in co-owning a housing unit, however, he shall have the right of seniority in the queuing for housing once he has completed the payment. Interested renters will also come from this queuing list.

ALLOTMENTS & ENTITLEMENTS OF FAMILIES with ISNs DESCRIPTION VALUE/annum 1 point = P 50.00 Psychoeducational Assessment/ Support Needs Assessment/ Career Preference Test/ Consultation/ Counseling Required as part of the Intake Process Required/ once a year / P 6,000.00 (120 points) Discuss this so the teachers would know their work opportunities

Service is provided to communicate needs of Recipients to Providers 2. Community Meals 1x/month; meal to 1 family member; points forfeited if not used; points cannot be accumulated for future use (to encourage regular attendance) P 3,000.00 (60 points) 3. Concierge Services Service is provided to communicate needs of Recipients to Providers Included in Community Fee 4. Community Activities Psychosocial B. Functional Academics Self-Care Vocational services/ Actual Job Performance E. Sports and Recreation 5. Spiritual Services Participation in religious celebrations within and outside the Happy Homes community Community Linkages Those encoded in red fonts apply to TEACHERS.

USE of MBH Facilities DESCRIPTION 1. Guest Rooms Upon availability , with accommodation fee 2. Social Hall (Great Room) year-round following house rules, with accommodation fee for events 3. Children's Room year-round following house rules 4. Teen's Room 5. Library/Art Studio 6. Teaching Kitchen for preparing community meals/ On Enrollment basis if to be used for the ISN’s Vocational Program /for Events These applies to TEACHERS as well
