Volume 141, Issue 1, Pages 253-258 (January 2012) An Unusual Pulmonary Mass With Mediastinal Invasion and Multiple Intrapulmonary Nodules in a 52-Year-Old Man  Chih-Jen Yang, MD, Jui-Ying Lee, MD, Chun-Chieh Wu, MD, Hsin-Ling Yin, MD, Chi-Tun Lien, MD, Yi-Chang Liu, MD  CHEST  Volume 141, Issue 1, Pages 253-258 (January 2012) DOI: 10.1378/chest.11-0360 Copyright © 2012 The American College of Chest Physicians Terms and Conditions

Figure 1 A, Chest radiograph showing a mass obscuring the right hilum. The triangular configuration along the lateral margin is consistent with volume loss in the right upper lobe. A widened mediastinum with loss of the right paratracheal stripe is consistent with extensive mediastinal adenopathy. In addition, multiple small nodules over both lung fields were noted. B, Chest CT scan revealed a huge lung mass in the right upper lobe, with lower trachea narrowing and displacement, mediastinal invasion, superior vena cava encasement, and multiple pulmonary nodules over both lungs. Large lymph nodes were noted over the prevascular, right supraclavicular, and paratracheal areas. C, Chest high-resolution CT scan showed pulmonary nodules in the left lung. Figure 2 A, PET scan showed an 18F-2-fluoro-deoxy-d -glucose (FDG)-avid mass, maximum standardized uptake value 19, size 6.3 cm, in the right upper lobe of the lung with superior vena cava encasement. B, Multiple high-grade FDG-avid nodules in bilateral pulmonary fields. C, One of the pulmonary nodules was sampled under video-assisted thoracoscopic surgery. Figure 3 A, The specimens showed multiple nodular interstitial infiltrates composed of Langerhans cells, exhibiting delicate folded nuclei with prominent groove and moderate eosinophilic cytoplasm in the background of interspersed eosinophils, histiocytes, and small lymphocytes (H&E stain, original magnification ×100). B, A high mitotic rate of >25 mitoses per 10 high-power fields was presented in H&E stain (×400). Arrows indicate mitotic cells. C, The immunohistochemical stain was positive for S-100 (×400). D, The immunohistochemical stain was positive for CD1a (×400). H&E = hematoxylin and eosin.
