Other states require above for the last 5 or 10 yrs. Most states require a description of the claim in the applicant’s own words. ANY inconsistency MUST be addressed by the applicant. State License Application Process Overall Process Continued The state licensing staff identify any verification not yet received – each entity must be contacted separately to have verification sent to the state board. The state licensing staff MUST receive the verifications directly from the entity. For example, the physician can not submit copies of transcripts from the medical school. State License Application Process Overall Process Continued Federation Credentials Verification Service (FCVS). Some State Boards are using FCVS to obtain primary source verification of medical education, postgraduate training, examination history, board action history, board certification and identity. This repository of information allows a physician to establish a confidential, lifetime professional portfolio. Delays lengthened if the physician has difficulty obtaining copies of “older” documents. Process is VERY structured by each state.
