Primary Care Mental Health Service Service offer Short term supportive Mental Health treatment interventions in Primary Care Delivered in Primary Care venues and patient homes Expert Mental Health advice and support to Primary Care Practitioners Target patient group Not ready for/or excluded from IAPT service offer. Complex and chaotic (e.g. if experiencing significant levels of distress and instability, misusing drugs or alcohol) . Do not meet the threshold for secondary care Mental Health services. Team Manager Clinical Lead & Non-medical prescriber 7 Band 6 Mental Health Nurses 1 Band 5 Mental Health Nurse 2 Peer Support Workers

IMPROVING PATHWAYS Service Design Referrals can be made directly from Opportunity Nottingham via Email instead of going through SPA. Allocated clinician each day to respond to Opportunity Nottingham referrals – contact to be made with the Opportunity Nottingham worker within 2 days. Flexibility in where and how individuals are seen – community locations / home visits if applicable. All referrals accepted for assessment – unless otherwise open to another secondary care team Beneficiaries of Opportunity Nottingham allocated to one clinician for consistency. Staff at PCMH service have access to INFORM – to allow information sharing Service has NMP to improve access to pharmacological interventions

Progress to date PCMHS began operating in Jan 17 We’ve had 40 referrals from Opportunity Nottingham Seeing Beneficarieis in flexible locations – Home visits/GP Practices/Val Roberts House Providing quick response and crisis support.

Challenges Promoting system change: - accessible locations - referral onto other services Staff attitudes/different ways of working - Staff expectations of a PCMHS - 2 different systems Service challenges
