Fatality - RTA Rollover – Private Vehicle HSE LEARNING INCIDENT: The deceased was on sick leave and decided to travel back to worksite on his private vehicle. At 14:30 hours near Rima roundabout he lost control of the vehicle and rolled over five times. He was taken to Rima clinic by ROP and then transferred to Haima Hospital where he was declared dead 30 minutes of his admission. ROOT CAUSES: Failure to follow Life Saving Rules in his private life (over speeding, not wearing a seat belt & use of GSM while driving). Failure to follow company policies (commuting) and instructions (doctor ‘s). LEARNINGS: Follow Life Saving Rules (wear seat belts, comply to speed limits, do not use GSMs). Never use private vehicles commuting to the interior. Follow company/doctor’s instructions. Key Learning: Follow company and doctor’s instruction.
