City of Fort Saskatchewan Safety Qualification Form The focus of this particular slideshow is Section 4 – “Safety Management System” City of Fort Saskatchewan Safety Qualification Form Thank you for choosing to work with the City of Fort Saskatchewan! This slide show is intended to help walk you through filling in the form and providing examples of what attachments will be required.

Section 4: Safety Management System Safety Management Systems, also know as “Safety Programs”, are currently not a mandatory requirement under Alberta OH&S Legislation. Having a SMS is however strongly encouraged and will become a requirement in the future for all City Vendors for employers with 20 or greater employees by 1 June 2018. Vendors with a Certificate of Recognition (COR) or Small Employer Certificate of Recognition (SECOR) for safety management may be given a preference over those without one. If you do not know what a Certificate of Recognition is, click here. Section 4: Safety Management System Do you have a Certificate of Recognition (COR or SECOR) for safety? 4. SAFETY MANAGEMENT SYSTEM Does your company have a COR or SECOR in the Province of Alberta? Yes No If “Yes”: Please attach a copy of the Certificate and table of contents. A copy of the SMS will be required after approval. If “No”: Does your company have an existing SMS that meets (SE)COR requirements? Yes No If “Yes”: Please attach a copy of table of contents. A copy of the SMS will be required after approval If “No”: Please confirm if you have any parts of a SMS: a. Corporate Safety Policy No Yes b. Roles and Responsibilities c. Hazard Assessment Process d. Planned Worksite Inspections e. New Hire Orientation f. Supervisor Training Program g. Worker Competency Program h. Safe Work Procedures i. Safe Work Practices j. Section 4: Safety Management System If you do not have a safety management system with recognition for COR or SECOR, check “No”. This does not remove you from consideration for work. If you have a Certificate of Recognition for health and safety management, check “Yes”….. ….and attach a copy or your COR/SECOR certificate…. ….and table of contents from the program manual…. This too does not remove you from consideration for work. If you do not have COR or SECOR but you are working towards it and have a SMS that meets COR or SECOR requirements, check here… Section 4: Safety Management System 4. SAFETY MANAGEMENT SYSTEM Does your company have a COR or SECOR in the Province of Alberta? Yes No If “Yes”: Please attach a copy of the Certificate and table of contents. A copy of the SMS will be required after approval. If “No”: Does your company have an existing SMS that meets (SE)COR requirements? Yes No If “Yes”: Please attach a copy of table of contents. Section 4: Safety Management System If you do not have a safety management system but you have some processes or policies for health and safety, tell us what you do have. Some areas are more critical to have and potential vendors must be willing to develop these areas or risk being declined a safety approval. Section 4: Safety Management System Hazard assessments are a must… If you do not know what to do to develop these areas, please contact your City representative for assistance. 4. A copy of the SMS will be required after approval. If “No”: Does your company have an existing SMS that meets (SE)COR requirements? Yes No If “Yes”: Please attach a copy of table of contents. A copy of the SMS will be required after approval If “No”: Please confirm if you have any parts of a SMS: a. Corporate Safety Policy No Yes b. Roles and Responsibilities c. Section 4: Safety Management System If you indicated that you use sub-contractors in Section 2, you should also have a process for screening and approving your sub-contractors similar to what you are doing with the City right now.
