e-Survey Overview

Overview of e-Survey Share Our Strength is moving to an e-Survey process for Cooking Matters curriculum e-Surveys will be fully implemented across all partners starting July 1, 2017 Share Our Strength’s Cooking Matters

What curriculum will use e-Survey: Cooking Matters for Adult and Families Store Tours, Pop-up Tours and Event Day Tours Available in: English, Spanish, French and Portuguese Share Our Strength’s Cooking Matters

Collection Methods Tablet Smart Phone Survey is completed offline Surveys will have to be uploaded after class Uses data plan or Wi-Fi if available at site Information is instantly uploaded Tablets are the preferred method GSFB will be providing 6 tablets to be shared among Nutrition Educators - multiple participants can use the same tablet to take the survey - as an alternative participants can choose to use their own smart phone to take the survey Share Our Strength’s Cooking Matters

Tablet Basics

GSFB will be providing tablets for the e-Survey Kindle Fire (5th Generation) 6 Tablets per Bin Each tablet is numbered Green Tablet Case Charger Additional Supplies Card with e-Survey links Power cord for charging Plastic case for storing and transporting The Kindle Fire will arrive set up with the Qualtrics Application which will be used to take the survey Share Our Strength’s Cooking Matters

Are found at the bottom of the screen Tablet Home Keys Are found at the bottom of the screen Will bring you back to the previous screen “Home” Button - Will bring you back to the home screen Will display all open applications on the tablet Share Our Strength’s Cooking Matters

Connect Tablets to Wi-Fi

Connect Tablets to the Wi-Fi When you receive your set of tablets you will have to connect them all to your office Wi-Fi Select Settings When you receive the tablets you will have to connect them to your Wi-Fi connection if available Share Our Strength’s Cooking Matters

Select Wireless Select Wi-Fi Share Our Strength’s Cooking Matters

Select your local Wi-Fi Network and enter the password You should now be connected to the Wi-Fi and will not have to complete these steps again; unless the Wi-Fi connection or password changes

Pre Class Steps/Prep Share Our Strength’s Cooking Matters

REMINDER: Complete and send in Course Scheduling Form When possible send form at least one month prior to the start of class All course paperwork can be found on the GSFB web portal https://www.gsfb.org/snap-ed/

this will be required to complete the e-Survey Get course code 7 digit number this will be required to complete the e-Survey is provided after Course Scheduling Form is received by GSFB Can be found on the manila Gift Card Envelope that you will receive from GSFB prior to the start of your class Check tablet batteries; charge if needed Share Our Strength’s Cooking Matters

4. Pack tablets and paperwork Tablets Course Code Attendance Sheet** Participant Waivers Volunteer waivers (if needed) Paper Surveys/Pens (just in case…..) Share Our Strength’s Cooking Matters

Attendance Sheet Should be using new 2017 Attendance Sheet Discuss importance of participant initials Can use up to 5 letters Is not case sensitive Must use same initials for pre and post survey Initials should be recorded on attendance sheet so participant has reference to what they used on pre survey Should be using new 2017 Attendance Sheet New column to record participant initials

Participant Waivers Waivers will still be completed on paper and kept on file Liability waiver is required Media waiver is optional Note on the attendance sheet if participants signed the waivers as this information will be needed to complete the end of course survey Share Our Strength’s Cooking Matters

First Class

Tablet Setup (before participants arrive) When you arrive at the class location turn on tablets using the silver power button on the top of the tablet Slide your finger across the screen to get to the home screen. Passwords are not required Select the Qualtrics Application which is on the home screen Add video Share Our Strength’s Cooking Matters

Select the appropriate survey for your class - Be sure to select “Pre” course survey Share Our Strength’s Cooking Matters

5. Select “Take Survey” Share Our Strength’s Cooking Matters

6. Change language if needed Language can be changed at any point throughout the survey Languages available include: English Spanish French Portuguese Share Our Strength’s Cooking Matters

7. Complete Required Information Required information includes: Course Code Partner Code Zip Code (use course location) Participant Initials Participant initials will be entered when they arrive at the class and is included in the next steps If these boxes are not completed participant will be redirected to RED boxes to complete before moving on to complete the survey Share Our Strength’s Cooking Matters

When participants arrive Ask participants their initials: Write initials on attendance sheet enter initials in the appropriate box on the e-Survey then select “Next.” Can use up to 5 letters Are NOT case sensitive 2. Hand the tablet to participant once survey questions appear on the “Next” screen Share Our Strength’s Cooking Matters

E-Survey is now ready for participant 3. Participants will complete demographics and survey questions At the bottom each screen they will select “Next” to move to next set of survey questions Reminder: just as with the paper surveys participants are not required to answer questions on the survey that they are not comfortable with. 4. Thank you page- Participants should show this page to Course Coordinator/ Nutrition Educator when complete Course coordinator/ Nutrition Educator should double check participant initials on the attendance sheet Share Our Strength’s Cooking Matters

5. Close the participant survey by selecting the X in the upper right hand corner of the screen Share Our Strength’s Cooking Matters

The tablet is now ready for the next participant to take the survey. Several participants can use the same tablet to complete the survey From this screen the Nutrition Educator will have to repeat steps to enter the unique identifiers: Course Code Partner Code Zip Code (of course location) Participant Initials Share Our Strength’s Cooking Matters

Smart Phone How To

1. Open a browser and type in appropriate e-Survey link Course specific e-Survey web links will be provided in each bin. Wi-Fi or Data Plan is required when using Smart Phone If Wi-Fi is available provide password If Wi-Fi is not available the app will use about 1MB of data plan for pre- survey (equivalent of uploading a high resolution photo or streaming 30 seconds of video). Survey uses about 1MB of data for pre-survey and less than 1MB of data for the post-survey Share Our Strength’s Cooking Matters

2. Provide required information for participant to enter Required information includes: Course Code Partner Code Zip Code (of course location) Participant Initials Share Our Strength’s Cooking Matters

3. Complete the e-Survey Select “NEXT” at the bottom of the screen to move on to the next set of questions Share Our Strength’s Cooking Matters

4. Show the final “Thank you page” to Coordinator/ Nutrition Educator Nutrition educator should record initials on the attendance sheet as shown in next slide Share Our Strength’s Cooking Matters

After First Class

This must be done within 48 hours of your first class Upload e-Surveys Each tablet used in class will have to be uploaded As Soon As Possible after the first class This must be done within 48 hours of your first class

Waiver Will continue to be signed in paper form and kept on file Participant will indicate which waiver(s) they signed by selecting appropriate bubble on the post e-Survey Share Our Strength’s Cooking Matters

The “FOR STAFF USE ONLY” section of the paper surveys is now captured within the e-Survey Share Our Strength’s Cooking Matters

After Last Class

Surveys should be uploaded As Soon As Possible after the last class Upload e-Surveys Surveys should be uploaded As Soon As Possible after the last class

Select the Qualtrics application on the home screen Select the appropriate course survey from the list Select “Upload Responses”

Once surveys have been uploaded the number of “recipient responses” should change to zero. “Uploaded responses” will then indicate the number of responses uploaded from that tablet. ***Denise will check course number to see that surveys were uploaded and indicate number of surveys uploaded to confirm with nutrition educator.

e-Survey Follow-up After the first and last class you will receive an email from Denise Gaudette (dgaudette@gsfb.org) to confirm the number of uploaded surveys matches the total number of participants. Please notify Denise if: a class is cancelled course dates have changed the number of uploaded surveys does not match the number of participants Any e-survey/tablet questions

Additional Information Share Our Strength’s Cooking Matters

Paper Surveys Are being phased out Paper surveys will not be mailed to Share Our Strength after July 2017 Can be used in extenuating circumstances - if paper surveys are used they will have to be entered manually via the tablet or web link Continue collecting paper surveys for any teens classes and submit as usual. The process for teen surveys will be discussed further at a later training. Tablet Distribution Denise will be at the May 10 SNAP-Ed training Please bring any questions; there will be time for Q/A during lunch to discuss Short demonstration with tablets after lunch Nutrition Educators will receive tablets on May 10 Tablets bins will be shared between Nutrition Educators (see next slide) June/July SNAP-Ed training calls will have time for e-Survey Q/A Share Our Strength’s Cooking Matters

Tips Tablet Check List will be available on the web portal. Participants are not required to enter personal info such as name, address, phone number and email. This information can be left blank as long as the required information (course code, partner code, zip code and initials) is entered in that section. When tablets are plugged in they automatically turn on. Do NOT leave tablets in your car in cold/hot weather; it will drain the battery.
