S M A R T SMART Model Specific Measurable Attainable Relevant Timely The goal is well defined and focused Develop a goal that is targeted at the intended area. Develop a goal that has clear intention – is your goal intended to monitor an outcome, improve an outcome, or predict an outcome. M Measurable The goal has a measureable outcome Can it be reasonably measured? Is the data practical to collect? A Attainable The goal is realistic Do you have control over any of the processes/ elements impacting the goal? Can an achievable target be applied to your goal? Do you have the resources to achieve the goal ( personnel, support ,money, etc.)? R Relevant The goal is based off current conditions and realities Does the goal support the desired outcomes you are trying to achieve? Does the goal support the strategic direction ? To help you develop your goals, we suggest you utilize the SMART model to help ensure you have quality goals for your HP program. You should find a handout in your packet that depicts the information seen here. This is a resource I’d highly encourage you to use and hang onto. The methodology is considered a best practice when setting goals and the concept will be useful for you throughout your career and life. T Timely The goal should be grounded within a timeframe Develop a goal that the data can be collected when needed. Can an achievable target be applied to your goal?
