Sample Individual Email (to communicate with them about Pilot) Title: Individual 360 Radar Pilot Hello! You have been selected to participate in an <Individual 360 Radar Title Here> assessment as a Pilot participant in our trial of AgilityHealth. The goal of the assessments is to dig deep and establish a baseline of how you and your peers feel you are doing. This is not meant to be a performance review, but rather a way to develop a clear path for growth which may include training, coaching, self-paced learning, mentorship, experiential practice and strengthening communities of practice. The final output is a Growth Plan with actionable items you want to achieve within the next six months and items you would like leadership to address as well. You will receive an email link to a survey from AgilityHealth, as will peers and stakeholders we have selected to also answer questions about how you are doing. We encourage you to be open and honest with all of your feedback. Provide specific responses in the text boxes provided and remember, feedback is a gift! A growth session with you and a facilitator will be scheduled to review your results together and help you develop your growth plan. We are launching Individual 360 assessments through AgilityHealth as a pilot with select individuals. You have been identified as a Reviewer for one or more of these Individuals, which means you will receive an email or emails on [DATE] with a link to a survey on how you think the Individual is performing. Please complete each survey you receive by [DATE] as this will allow the Individuals to review the results during their growth session. Your input is anonymous and is a critical part to the overall assessment results and individual growth plan. Sample Continuous Improvement Session Invitation (to Schedule Continuous Improvement Sessions with Individual) Subject: AgilityHealth Individual 360 Continuous Improvement Session Hello! Thank you for participating in our Pilot of AgilityHealth and their Individual 360 Radars. We are scheduling a Continuous Improvement Session with you now, to review your radar results and to develop a clear path for growth which may include training, coaching, self-paced learning, mentorship, experiential practice and strengthening communities of practice. Desired Outcomes: Analyze your radar and engage in an open/honest conversation about your strengths and growth opportunities. Develop an actionable continuous improvement plan for you to achieve within the next six months. Let your leadership know where they can help you. Tips When Analyzing Your Results: You should have received an email from AgilityHealth asking you to set up your account. Once logged in, you’ll see the Summary View of your results, in addition to the Detailed View with plotted dots for the responses. At first glance, what is the overall radar telling you? Is it generally rounded with a few dips or do you see many dips and spikes? Are you stronger on one side vs. the other (left vs. right)? How did you rate yourself compared to what others have rated you? Are you mostly higher than their average or mostly lower or on target? What does this mean? Review the Top/Lowest 5 Competencies table. What does this data tell you? Review the Top/Lowest 5 Consensus table. What does this data tell you? Now review the textual responses, spend time on the Strengths comments and take a min to really focus on what you’re doing well. Go down and read the improvements and possible impediments. Keep a positive growth perspective to all of this and remember the most important message of all ‘FEEDBACK IS A GIFT’. Review your results prior to the session. We’ll talk through them together and will build your growth plan from there.
