Sharing the Learning from Deaths in mental health driver diagram 2018-19 AIM PRIMARY DRIVERS SECONDARY DRIVERS CHANGE IDEAS Aim To equip 100% of organisations t present at least 1 piece of work that demonstrates shared learning from deaths which uses QI methodology by July 2019 Capability Building 3 Ls Programme Connecting in active periods Signposting to QI resources Collaboration Connecting people Create structure to enable people to share Shared Common Purpose Use expertise All teach All Learn Creating a Culture for Shared Learning Enabling Psychological Safety Create Joy at Work Co-production with people who use our services and their families National voices Create Infrastructure to enable people to engage with collaborative Menu of opportunities for co-production Pre-learning for events 3 L sessions at Learning Sets – systems thinking Visit Teams at bases Webinars Teleconferencing PM meetings Coaching sessions Attend local QI events Linking into Q Create video storyboards Free online courses Good QI resources – including Life System Social media Identify key learning themes Connecting with all relevant stakeholders –primary care/acute trusts/social services etc Use world café, storyboards, website, videos etc Connecting through social media Create fora to share – LS, Life, website Engage with national leaders and bodies Engage with colleagues in leadership, QI teams, delegates and at induction Listening and learning – You said – we did Ensure shared understanding of psychological safety Modelling behaviour Joint work SCORE survey Engagement with national voices - Making families count, SOBS, PAPYRUS Put in place formal agreement of structure for co-production to include opportunities expenses, safe space and support All organisations to be asked to bring 2 people MHC Faculty April 2018
