Figure 1 (A) Portal tract expansion with bile duct/ductular proliferation, especially evident at the limiting plate (arrowheads), is noted in this liver biopsy from a 6-week-old infant with BA. Biliary pigment is noted in some ductules (arrows). There is also a cellular infiltrate in the portal spaces that appears to be largely extramedullary hematopoiesis. The surrounding lobule shows little change, except for mild extramedullary hematopoiesis and focal hepatocellular multinucleation. This was rated as consistent with BA by 10 of 10 pathologists. Portal tracts are mildly expanded by a cellular infiltrate, with only minimal bile duct/ductular proliferation. Only 2 of 9 pathologists favored diagnosis of BA.
