Evaluation Dr. Gregory Wiens (HGC) visited the Mid-year GA Meeting - March 2011. HGC leadership (Jeannette Flynn, Ken Love, Dan Holland & Greg Wiens) directed an evaluation weekend on June 10-12, 2011. Focus group meetings in three locations - Friday. Interviewed the Ministry Council on Saturday. Saturday afternoon, shared life-cycle concerns to all. Attended four congregations (preached at two). At least 20 congregations were represented either through focus groups/interviews. 9/24/2011

Proposal Adopt a clear and concise mission. The CDP District exists to produce healthy and growing churches that are transforming their communities, nation and world for Christ. Develop a Church Transformation Ministry. Turn plateaued and/or declining churches toward effective mission and community impact. Target six churches over the next two years. Create a church multiplication strategy. Plant three congregations in the next three years. Follow the PATCH process followed by HGC. (Prescreening/Prayer; Assessment; Training; Covenant; Help through Supervisor, Coach; Mentor; Peer Group; Trainer; Therapist) 9/24/2011

CDP District pay 25% ($1250) per consultation, with the church covering the remainder ($3750). Implementation of six covenants. Six supporting coaching relationships (two years) 9/24/2011
