Shared Care Raising Awareness Presented by Tania Barnes – Shared Care educator Sonia Lee – SHAREHD Programme Manager Programme Supported By

The Vision For people who receive dialysis at centres to have the opportunity, choice and information to participate in aspects of their treatment and thereby improve their experience and their outcomes.

‘Shared’ Doing something with rather than at or to Taking a small or large part in something with other people

All patients, All areas, All of the time ‘Vision’ ‘Martini’ effect Offered & available to all, & meaningful to the individual one task * part of task * many tasks All patients, All areas, All of the time

Choice? Sharing your care = a choice because: It’s a change in the way we have traditionally cared for our patients (Can’t just change the rules) We create great resistance when we tell people what to do and what is good for them Choice is important because it allows individuals to develop : in their own time at their own pace when they are ready Everyone is different and the way patients deal with their lives and cope with their dialysis treatment needs respecting if we are delivering compassionate care

Respect People want to do shared care for different reasons Everyone else is doing it! I would like to have a go at just a little bit I want more control I want to go home to dialyse I would like to be busy rather than wait I want to be useful I am interested in learning certain bits I want to go on holiday and feel more confident I like being involved it makes it more interesting I might feel better Not many people will say that it will lead to me having better outcomes!

Choice/Policy Have to = handwashing, unit policy, that’s the rule because…! Opportunity - e.g Introducing a new piece of technology – when something is imposed, use it to maximise engagement

Influencing choice Seeing is believing – “If they can, I can” The trick lies in how we can influence patients and staff so that it remains their choice using their own motivation to become engaged e.g. Seeing is believing – “If they can, I can” Peer to peer conversation - “ hmm, that’s interesting, he/she is like me” Offering the opportunity – “I didn’t know I could do that”

What message best encapsulates Shared HD Care for you ? We want your help What message best encapsulates Shared HD Care for you ? 5 Mins

Emphasis is on choice, taking part and engaging at a level that suits the patient as an individual.
