SAMPLE Defining our core We are a committed collaboration of stakeholders. We are united in improving the lives of youth with disabilities beyond high school. To accomplish this we use an active and consistent interagency approach that includes all stakeholders. We will know that we are making progress when our change in practice results in a measurable increase in youth outcomes. (South Carolina Community of Practice on Transition, 2013)
