These are the forms from Volume II of the Proposal package that will be reviewed. Remember that all forms must be filled out in their entirety, where applicable. All forms must have original signatures and original seals as well. Step 2: DOP will submit ONLY the responsive proposals to Internal City Stakeholders to review for responsibility. And those stakeholders are in the Office of Contract Compliance, the Office of Enterprise Risk Management, and the Dept. of Watershed Management.
