Cohort Participation Expectations Members will write an AIM Statement Members will set at least one goal to report Members will participate in 3 face to face meetings Members will participate in follow-up conference calls every 2-3 weeks Members will report progress, challenges, barriers, lessons learned during conference calls Members will present final achievements during Learning Collaborative in August 2015
