Mu Alpha Theta August 15th

Requirements: Mu Alpha Theta Math Honor Society Junior or Senior Overall GPA of 3.0 or higher 3.5 Mathematics course average in accelerated courses, or 4.0 in on-level courses.

Please complete the application below according to the guidelines listed and return the completed application to Mrs. Anderson (Room 642) no later than August 31,2018. The teacher recommendations must also be in by that day, so please give your teachers plenty of time to complete those. A one-time fee of $42 will be required and should be submitted once you are notified whether or not you have been accepted. This fee can be paid with a check made out to SFHS. Please do NOT bring cash. If you bring payment before you are notified about acceptance, it will not be taken. The fee pays for your national dues, local dues, your graduation cord and your t-shirt.

Tutoring options this year: Monday & Tuesday afterschool in Room 642, 3:45-4:15 During IF, sign-ups will be on ItsLearning. You must give Mrs. Anderson 24 hours notice (unless sick) if you cancel. Otherwise you will not be allowed to tutor during IF again. Saturday Academy (Dates/Times on SFHS Website) Tutoring outside of school-Must be signed by adult Tutoring of another SFHS student must be done in the presence of a teacher to count.
