Inpatients with diabetes on insulin: self-adminstration pathway Daily diabetic foot check on all diabetic inpatients by Diabetes Team During foot-check confirm if patient takes insulin YES NO Is patient able to self administer insulin? Does patient need to be reviewed by Inpatient Diabetes Specialist Nurses? Complete PHQ-9 and questionnaire (re self-administration of insulin) END of pathway (refer to other diabetes pathways) YES NO YES NO Ensure correctly prescribed on drug chart Ward staff to complete Diabetes Referral on EPR Give patient pencil case to store their own insulin pen devices Ward staff nurses to continue administering insulin and document on EPR at time of administration Patients to store needles separately (for safety) to self-administer insulin – and dispose of sharps Insulin to be stored in patient’s bedside locker to be administered by ward staff with safety needles Ward staff nurses to document on EPR date/time insulin was self-administered
