Again: What is a silent deviation? Deliberate or not? What characterise procedures we break? Accepted by all or by some? A silent deviation is a breach of a procedure that has developed into accepted practice

This far … Silent deviations 1: What deviations do we have? We have many silent deviations All of them have serious potential, some more than others Silent deviations 2: Why do we have them? We do it because it is convenient, and everybody does it like that We do not interfere. Usually it turns out ok. We hope that goes for this time too Silent deviations 3: What can we do to stop them? We have 3 demands to the organisation / management We have 3 demands to ourselves A silent deviations report will be made available for all after session 6

Now we like them to stop Reducing and / or eliminating silent deviations must be done on all levels: Organisational / management level Crew / practical level Personal level
