Vision Insurance Vision insurance is through the Health Trust Vision provider is VSP Rates are the same this year as last year No plan changes
Dental Insurance In order to avoid an almost 12% increase in costs Our Dental insurance is now through the Health Trust Only one Dental plan available We still go through Delta Dental and have the PPO and Premier Network available
Dental Insurance Important Plan Changes The Health Trust allows you to carry your Delta Dental into Retirement (Effective February 1, 2019) Annual Benefit maximum is $1,250 (the old plans were $1,000 and $1,500) Annual deductible is $25 per person/$75 per family (The old plans were $75/$225 & $100/$300 Annual deductibles) No double up Maximum available with this plan Orthodontic coverage is on your plan with 50% coverage (same percentage as our high plan previously) Last years rates High Plan Single - $12.51 2 People - $21.57 Family – $37.41 Low Plan Single - $10.49 2 People - $17.88 Family - $29.64

Health Reimbursement Account PPO 500 $1,200 for Single PPO 500 $2,400 for Family PPO 1500 $1,700 for Single PPO 1500 $3,400 for Family
Flexible Spending Account- Medical The maximum contribution this year is $2,900, (This number includes whatever the city is contributing towards your FSA account) FORM REQUIRED! (Please print your name on the forms in clear writing!)
Flexible Spending Account - Dependent Care The city does not contribute anything to this benefit If you are single or filing separately the maximum you can contribute to your dependent care account is $2,500. If you are married and filing jointly, the maximum you can contribute is $5,000.
MMEHT Income Protection/Life Insurance Income Protection and Life insurance rates are not changing You can apply/cancel/change at any time, if you are increasing coverage you are required to fill out an evidence of insurability for this and you can be denied based on medical history
Retirement Health Savings Account You have an Retirement Health Savings account through ICMA These funds can only be used upon retirement to pay for medical expenses You cannot contribute to this account on your own at this time, this is specifically only unused vacation/sick days per your contract
457 – Retirement Account Can start/stop/change at anytime Go through 3 different vendors: ICMA, Mass Mutual and MESTART The limit for the 2019 calendar year is $19,000 (This rate does not include the Age 50 catch up provision or the Pre-Retirement Catch up Provision) 6,000 for age 50 38,000 pre retirement catch up prevision
REMINDER REQUIRED FORMS Dental Form FSA Form Because we are going through a new administrator (MMEHT), if you want any sort of Dental coverage, you must fill out this form FSA Form You need to fill out this form even if you only want city paid coverage offered If you want to add your own coverage, even if you had coverage the previous year, you need to fill out form again
