Leading Transformation Change for Health and Social Care

What Matters to Me? May Connor is a 68 year old woman who has been diagnosed with dementia. She cares for her husband. Phil Connor is 71 years old and has been married to May for 41 years. He had a stroke 2 years ago and has relied heavily on May ever since. He is also diabetic. Jean Collins is May’s social worker (only heard on the phone). Belinda is a hospital nurse. She is worried about sending May home but is also under pressure to free up beds.
