Figure 17D Further examination revealed focal areas with numerous eosinophils in the areas of edema and infiltrate, in addition to the lymphocytes and plasma cells (Fig 17D, hematoxylin and eosin stain, original magnification ×400). These findings are diagnostic of a superimposed acute interstitial nephritis, with the numerous eosinophils strongly suggesting hypersensitivity-induced etiology. This diagnosis was also verified by the clinical follow-up, where removal of the likely offending agent (in this case ciprofloxacin) led to return of creatinine to baseline levels.
