HSE LEARNING INCIDENT: Floorman was running in drill collars when he pushed the tongs towards the drill collar without holding them by the handles. The jaw made contact with the drill collars and bounced backwards pinching his finger. Pictures on the next slide CAUSES: Improper placement and handling (floorman’s hands in wrong place and not pulling down and pushing over the tongs in separate steps) Failure to warn/intervene (no one intervened and stopped the floorman) Inadequate job/work preparation/planning (no TBT was conducted by supervisor at rig floor) LEARNINGS: Always hold safe/green handles while operating manual tongs. Tongs are meant to be operated by one person. If two then ensure proper communication Ensure all pinch points are clearly identified by proper color coding as well as discussed during TBT. Ensure that effective TBTs are conducted using TRIC.
