Where we started from…Dec 2015 General Practice (EMIS) Community Nursing (EMIS) MDT Coordinators (EMIS) Sunderland Care and Support (EMIS) A number of disparate systems = no sharing Social Care (Swift) Acute (Meditech)

Challenges No culture of sharing data outside of organisational boundaries or systems in Sunderland Concerns over Information Governance Concerns over levels of sharing – all or nothing Concern that a solution would be costly

Digital Solutions Start conversations with key stakeholders – understand and identify issues to progress Identify IG advisor for the programme – local and national Discussions with suppliers to identify solutions to introduce more granular sharing Use what we have (current capabilities)

The Journey Establishing the Information Sharing Group IG knowledge and Support Involvement of Primary Care, specifically Practice Managers Representation from all involved organisations including suppliers Comms and Engagement – Staff Email, Meetings, Practice Visits, TITO Benefits of a Phased Roll out Comms and Engagement – Patients Posters, Leaflets, News articles, Website, Digital screen messages.

Sharing Agreements Work through the detail! Can’t underestimate importance of practice manager involvement, including part of the Digital Solutions Team Information Sharing Gateway - electronic sign up - faster, easier process than paper - initial work to set up - becomes ‘business as usual’ for practices

What we now have… EMIS to EMIS Sharing Sharing between Sunderland General Practice, STFT Community Nursing, Sunderland GP Alliance MDT Co-ordinators and Sunderland Care and Support Reablement MIG GP Practices sharing regionally with: City Hospital Sunderland NDUC NEAS NTW Northumbria Healthcare Newcastle Hospitals South Tyneside FT County Durham & Darlington South Tees FT GATDOC Tees Esk & Wear Valley FT Each one is a separate agreement on the gateway for the practices to sign off More importantly we have a culture of sharing that we can build upon

Next steps for sharing… GP practices sharing EOL data: CHS – Agreement signed and with Healthcare Gateway and Meditech NEAS – Agreement to be set up on ISG NDUC – To be discussed at Information Sharing Group GP practices sharing frailty data: CHS – specialised data set to be agreed
