LONG TERM CONDITIONS AN OVERVIEW Alison Tongue WORKFORCE CHANGE IN Abbreviations- LTC – Long term conditions NWW – New ways of Working Team MA – Modernisation Agency DH – Department of Health NWIPP – National Workforce Information and Planning Programme CNO – Chief Nursing Officer SAP – Single Assessment Process Programmes within the Modernisation Agency including Changing Workforce Programme and Long Term Conditions team are working closely together with the DH to try to bring together all the above pieces of work that relate to the development of the workforce in long term conditions. This will culminate in a rapid roll out programme of workforce change with selected sites, ( 7 in total), based on SHA geographical regions from November 2004 until end of March 2005. It is envisaged that this work will continue into the new MA. Sites will be given pump priming monies to appoint project managers who will be equipped with appropriate information and techniques on workforce development and role design by the programmes identified above. They will then work with their local health and social care strategic communities, through exisitng networks, to identify what their existing workforce / skills gaps are and plan changes that can be made to meet those gaps. They will be asked to measure and evaluate their work and report to the MA and the national reference group on progress and outcomes. This will then be disseminated to the DH and Long term conditions care group workforce team as well as to other regions to quickly share good practice, identify key national issues particularly in relation to the development of national competency frameworks and the commissioning of appropiate education and training. This work will also be informed by the national developments around the skill sets for community matrons and case managers and the long term conditions policy framework to be produced by the DH , in which workforce is a key element.

Delivery of LTC; “ will require the workforce from across the multi-disciplines of Health and Social Care to be modernised and redesigned to ensure that all three levels of the Kaiser triangle are populated appropriately with the right skills to meet the needs of redesigned patient pathways. The aim is to facilitate change in the design of local services and workforce and implement new ways of working.
