DSRIP Project Plan must contain Category Project/Intervention Performing provider Description of intervention Goal Expected outcome Baseline data for intervention (patients served, percent of providers in intervention, number of encounters) Inter-related projects in DSRIP plan Table outlining: milestones, metrics, estimated DSRIP funding; estimated state match.

DSRIP Menu of Projects Draft State DSRIP menu of project options released April 5. Categories 1 (Infrastructure development) and 2 (Program innovation/redesign) – may choose within each category, projects not mandatory, pay for performance (must meet metrics to get paid). Inpatient or outpatient. Category 3 (Quality improvement)– Some items will be mandatory; others may be selected; participants are paid for reporting initially, then paid for performance (must meet metrics to get paid). Inpatient focused. Category 4 – (Population-focused improvement) – No choice of projects; must report on all; participants paid for reporting only. Inpatient focused.
