Presentation transcript:

“ Thanks so very much for your compassion and empathy as well as sharing your knowledge and expertise. ” Cindy called Health Advocate after her father was injured in a fall. When her father returned home from the hospital, he demonstrated cognitive and physical issues that she could not manage. Cindy’s Personal Health Advocate told her to take him to his doctor for evaluation, after which he was admitted to the hospital for more treatment. Cindy’s Advocate also spoke with her about her father’s medical coverage, costs, and resources like Veterans Affairs, and helped her find placement in a nursing facility. Upon his release from the hospital Cindy worked with her Advocate to discuss discharge planning.
